One last piece of the puzzle... I had to set the "remote" field in
.mythtv/lircrc to the name of the input device file. E.g.:

begin
     remote = /dev/input/ira
     prog = mythtv
     button = KEY_MUTE
     config = |
     repeat = 0
     delay = 0
end

I used mythbuntu-lirc-generator to create the .mythtv/lircrc. It reads
/etc/lirc/lircd.conf and "remote" corresponds to the "name" field in
/etc/lirc/lircd.conf, which is otherwise not used.
