42 degC is very low for a passive heatsink card.

Does the adaptive clocking function seem to work for you ?

And does it ever drop from maximum after running mythtv with playback
(FE) ?

I don't think have seen this work from before 195.xx or earlier.
Maybe dual displays forces this to max. 

My GT240 with passive heatsink & low air flow idles 61 degC at indicated
max clock. 69 degC TV1 playback.
