So to summarize what I think you have told us so far:
a)	You recorded a programme which created a file at:
	/var/lib/mythtv/recordings/1003_20120926180000.mpg
b)	When you try to watch the programme you get the error:
	GetPlaybackURL: '1003_20120926180000.mpg' should be local, but
	it can not be found.

It sounds to me like a mismatch of the frontend with the front end.

Are you using storage groups?
Are you using streaming the programme from the BE to the FE? or are you 
using NTFS to directly access the file?
Are you using a local (to the BE) FE or a remote FE?
Have you run the find_orphans script to see what it reports?

Maybe researching some of these questions will reveal the issue you are 
facing.
