I installed one as part of an upgrade in a friends myth box yesterday.

They come with a dual width low profile bracket and you move the DSUB
output down next to the DVI. I didn't use the low profile bracket but
the heatsink is larger than it looks in that picture - higher than
the bracket - and would prevent you from putting it in a low profile
case by the looks.
