Which version of the ivtv driver are you using ?

0.4.0 needed the files in /lib/modules

0.4.1 and newer need the files in /usr/lib/hotplug/firmware.

I've been using 0.4.0 under Ubuntu with no issues, and 0.4.2 under
Knoppmyth.
